While you do not have a lot of options when it comes to how your companion will look like, Pet Buddy Mod does let you customize it to your preference. All you need to do is to equip an item in its armor and it will take on a unique look. For example, if you put in a cookie, what you get is a pet that looks like a bat that flies. Adding a bone to its armor will make your buddy look like a skeleton. However, if you want it to be a mini-me, then leave out the last item.
